Help With the Costs of Living With a Movement Disorder
A specialist may be hours away, or the equipment you need may cost more than your family can manage right now. WVMDA’s grant program helps patients and caregivers in West Virginia, eastern Kentucky, and southern Ohio request assistance with some of these expenses. How a WVMDA Grant May Help
The expenses that come with a movement disorder are not always covered by insurance.
Even when they are, copays, travel, and equipment costs can still leave families with more than they can afford.
WVMDA considers requests for expenses such as:
- Travel to medical appointments and procedures
- Medical and medication copays
- Physical therapy
- Power chairs and scooters
- Other mobility-related equipment
- Accessibility ramps for the home
- Other expenses related to receiving care or maintaining mobility

Who the Grant Program Serves
WVMDA serves people with movement disorders in West Virginia, eastern Kentucky, and southern Ohio.
The program was created to give patients and caregivers in our area somewhere local to turn when
the cost of care, travel, therapy, or equipment becomes difficult to manage.
WVMDA works with people affected by Parkinson’s disease, essential tremor, dystonia, multiple system atrophy,
ALS, Huntington’s disease, Rett syndrome, multiple sclerosis, Tourette syndrome, and other movement disorders.
How to Request a Grant
1. Download the Form
Use the link below to open the patient grant request form.
2. Complete the Request
Answer the questions on the form and provide the requested information about the patient, the expense, and the assistance being requested.
3. Submit the Form
Once the form is filled out, use the contact information provided on it to send your request to WVMDA.
What Happens After You Apply?
WVMDA reviews each request using the information submitted with the application. Grant assistance is based on the individual request and the funding available at that time. Submitting a form does not guarantee that a request will be approved or funded.
